Before Your Appointment: Important Next Steps
1) Activate Your ChARM Health Portal
To access intake forms, messages and future lab results, activate your ChARM Health Portal using the registration link sent to your email. If you need assistance, call our front desk at 612-440-7710 and leave a message.
Important: If you’ve used ChARM PHR with another provider, log out before clicking the activation link to connect with our clinic.
2) Complete Your Intake Forms
Your intake forms are located in the ChARM portal under “Questionnaires.” Please submit these at least 2 business days before your appointment to ensure the best care experience.
3) Upload and Share Previous Lab Results
If you have lab results from the past 12 months, upload them to your ChARM portal under “Documents” and click “Share” after uploading.
If you don’t have access to past results, request your healthcare provider to fax them directly to Be Well Natural Medicine at 1-833-817-5943.
Please note: If we have not received your completed intake forms and previous labs at least 48 hours prior to your appointment, you will be contacted to reschedule your appointment in order to give your provider sufficient time to review your health history.

We accept Flex Spending (FSA) and Health Savings Accounts (HSA).
Insurance Information
We are out-of-network for all insurance plans. However, we can provide documentation if you wish to submit a claim for reimbursement. Let us know before your visit if you plan to do this.

Your visit will include:

✔️ Nutritional assessment & foundational dietary recommendations
✔️ Holistic assessment and personalized treatment plan
✔️ Recommendations for additional lab testing (if needed)
Between-Visit Communication
Our providers are available to answer brief follow-up questions via our secure patient portal (no email or voicemail).
For new prescriptions, case analysis, or complex concerns, you’ll need to schedule a follow-up appointment.
Cancellations & Rescheduling Policy
Please provide at least 24 hours’ notice if you need to cancel or reschedule.
Late cancellations (less than 24 hours) will be charged a $75 fee to ensure availability for other patients.
